to Lambda Theta Alpha. Please send an e-mail to Caroline
Bizot, National Director of Finance, for the mailing address. Please
submit the Name of the Sister, Chapter the Sister pledged and semester the Sister
pledged for EACH nametag you order.

Order Terms: All prices include shipping and handling.
You must submit payment at the time of order. Payment can be made in the form
of money order, chapter or area check, or via PayPal. PayPal purchases will
incur a nominal PayPal fee. Once payment is submitted, it is non-refundable
unless the item is out of stock. Please include mailing address and appropriate
customization information when submitting your order. Incomplete orders will
not be processed. Please allow three to four weeks for shipment.
